IPAT MEMBERS - Earn  $5.00 off of YOUR  next year's dues for each new 2007 member that you get to join IPAT with the form below:
LIMIT:  Each member may earn up to $20.00 total off of next year's dues if they bring in  4 new  2007 members.
This offer does not pay in cash.  It will only apply toward 2008 dues and only toward one (1) sponsoring IPAT member's dues.
This offer starts April  1, 2007 and will run through December 31, 2007
Please copy and print the following for any members that you wish to join IPAT as this is the only form that can be used to credit you with having them join.   If you call in, be sure to tell the name of the sponsoring member and their IPAT number.
